Affidavit of Intent to Home School Instructions


Please follow the directions for filling out the Affidavit of Intent to Home School Form carefully. Incomplete or incorrect information on the affidavit will cause a delay in registering your child for home schooling.

 
The affidavit is not complete without the required proof of your child's age and identity.
 
Child's legal name: Fill in your child's legal name. If your child's legal name is not the same as the name on the birth certificate please enclose a copy of the court papers documenting the name change.
Date of birth: Be sure to include the month, day and year your child was born.
School district of residence: The residence of the person having legal custody of the child is considered the residence of the child. Find out which school district you live in here.
Name(s) of the parent(s) or person(s) with custody of the child: Fill in the name(s) of the parent(s) or person(s) with legal custody of the child. If the parent's name on the birth certificate has changed please enclose court papers documenting the name change.
Daytime telephone number(s): Include a phone number where you can be reached during the day.
Physical address(es) of the person(s) with custody of the child: Fill in your home's physical address.
Mailing address: Include your mailing address if it's not the same as your home's physical address.
Signature of child's parent or person with custody: This signature must belong to one of the parents or person with custody listed above. The affidavit must be signed in the presence of a notary.
 

Arizona Revised Statutes § 15-828 requires you to provide proof of your child's age and identity. You may use your child's original birth certificate or an acceptable photocopy of the birth certificate for this purpose. We will return original birth certificates after processing your child's affidavit. If you are unable to provide a birth certificate you may use another document, such as your child's baptismal certificate, an application for a social security number, or original school registration records, as proof of age and identity. If you use a document other than the birth certificate you must also submit an Affidavit of Other Reliable Proof explaining the inability to provide a birth certificate.
